When you come to the clinic, our receptionist will ask you the following:
- Bring your Personal ID
- Your insurance card – we will make a copy and will keep
it with your records.
- The referral/prescription from your care provider for
Physical Therapy.
- Any applicable co-pay.
On any subsequent visits, it is important for you to keep us informed of any changes that may occur such as change in insurance, a change in address, a change in physician, or any other pertinent information.
We may also need to ask you to change into a patient gown for your evaluation and treatment. If you would be more comfortable in your own shorts and tee shirt or tank top, please bring them with you.
During this initial evaluation, the physical therapist will examine you and will establish a plan for further treatment and/or a home exercise program. All of our therapists are trained in manual therapy techniques to assist in your recovery from injury or dysfunction.
This means your Olympic Physical Therapy therapist will spend some time with you palpating for joint movement, tissue restrictions and disorders. They will use these manual skills to access your problem and care. Your therapist will develop a treatment plan that will include your own personal goals, as well as those of your physician and physical therapist. These goals will be reviewed at each visit and will likely include an exercise program that is individually designed to enable your recovery.
